Perimenopause Symptoms at 42
Approaching the typical window
At 42 perimenopause moves from possible to plausible. You are two or three years short of the average onset, and a meaningful minority of women are already in the early transition by now. Symptoms at this age are frequently misread as burnout, because the headline features, exhaustion, irritability and poor sleep, overlap almost perfectly.
What periods typically look like at 42
Look for cycle length changing by a week or more, in either direction, or flow becoming unpredictable. The formal marker of early transition used in research is a persistent difference of 7 or more days between consecutive cycles.

What is not typical, and needs a doctor
Exhaustion is also the signature of thyroid disease, anaemia, sleep apnoea and depression. If fatigue dominates at 42, ask for thyroid function and ferritin blood tests rather than assuming hormones.
Testing and diagnosis at 42
Between 40 and 45, NICE guidance says a doctor may use an FSH blood test to support the picture, alongside your symptoms and cycle history. A single normal result does not rule perimenopause out, because hormone levels swing widely during the transition. Menopause arriving before 45 is classed as early menopause and is worth confirming, since guidelines recommend considering hormone therapy until at least the natural age of menopause to protect bones and heart.
You can still conceive during perimenopause. Pregnancy remains possible until you have gone 12 consecutive months without a period, so contraception still matters at 42 if you do not want to become pregnant. An irregular cycle makes timing unpredictable rather than impossible.

Is constant tiredness at 42 a perimenopause sign?
It can be, especially combined with cycle changes, night sweats or mood changes. But fatigue alone has many common causes at this age, including thyroid problems and iron deficiency, so it should not be pinned on perimenopause without checking those first.
How long would perimenopause last if it starts at 42?
SWAN research found the transition lasts longer when it starts younger, often over 7 years in that case, compared with roughly 4 years on average. That is a reason to build sustainable symptom management early rather than white knuckling it.
